- Include both of the following statements in the body of the Notice:

- Identify the specific URLs or other unique identifying information of material that Youngboy Goes Dark Social has removed or to which Youngboy Goes Dark Social has disabled access.
- Provide your name, address, telephone number, and email address.
- Include the following statement: "I consent to the jurisdiction of the Federal District Court for the judicial district in which my address is located, or if my address is outside of the United States, for any judicial district in which Youngboy Goes Dark Social may be found, and I will accept service of process from the person who provided the original notification of alleged infringement or an agent of such person."
- Include the following statement: "I swear, under penalty of perjury, that I have a good faith belief that the material identified above was removed or disabled as a result of a mistake or misidentification of the material to be removed or disabled."

Upon receipt of a valid counter-notification, Youngboy Goes Dark Social may restore the removed material. If we do not receive notice from the original copyright claimant that they have filed an action seeking a court order to restrain the subscriber from engaging in infringing activity relating to the material on our system within 10-14 business days of our receipt of the counter-notification, we may restore the removed material.
